Job Description:
A company is looking for a Quality of Care RN -
Interventions.
Key Responsibilities:
Conduct outreach calls to members with gaps in preventative care
and educate them on its importance
Evaluate health-related social needs and ongoing care management
needs, making referrals as necessary
Participate in quality workgroups focused on HEDIS, Stars, and
other quality measures while documenting findings
Required Qualifications:
Associate's degree in a general field of study or post high school
nursing diploma
2 years of experience as a practicing RN in a hospital or
healthcare setting
Active, current, and unrestricted license to practice in the state
of Massachusetts
Experience in the health insurance field is preferred
Successful completion of pre-employment background check
