Job Description:
A company is looking for an RN Care Coach- Bilingual.
Key Responsibilities
Serve as the initial contact for the coaching team, guiding
caregivers and consumers through the referral process
Coordinate and complete assessments and consumer service activation
while ensuring compliance with procedures
Provide ongoing coaching and clinical consultation to caregivers
and the coaching team
Required Qualifications
Active Registered Nurse (RN) License with compact licensure
Minimum two years of recent experience in direct care of elders or
adults with disabilities
Experience in case management, care planning, and assessment in a
healthcare setting
Proficiency in common software and web-based applications
Fluency in Spanish, including reading and writing
