Job Description:
A company is looking for a Palliative Care Registered Nurse (RN)
to deliver evidence-based care to seriously ill patients through
telemedicine.
Key Responsibilities
Manage patient care and coordinate with the interdisciplinary
team
Communicate with patients through phone and video conferencing,
providing education and follow-up
Facilitate care coordination activities and manage patient
medications and supplies
Required Qualifications
Current CA RN license, with the ability to obtain additional state
licensure
1+ year of experience in palliative or hospice care
Minimum of one year experience as a professional nurse within the
previous three years
Willingness to work Monday-Friday from 8:30am-5:00pm in the Pacific
time zone
CHPN certification or equivalent preferred
