Job Description:
A company is looking for a Care Manager - Episodic UM to plan
and manage behavioral and physical care for members.
Key Responsibilities:
Advocate and educate members on services and health benefits
Conduct assessments to identify barriers and develop care plans
Coordinate care with multidisciplinary teams and document all
encounters
Required Qualifications:
NYS RN or LCSW/LMSW (any state) for Medical Care Management
3 years of experience in mental/behavioral health or addictions
setting for Behavioral Health Care Management
1 year of pediatric clinical field experience for PEDS
positions
Credentialed Alcohol and Substance Abuse Counselor for CASAC
positions
Experience managing member information in a shared network
environment
